The PARENT/GUARDIAN of the student in need of the Bus Pass MUST be the one to fill out this form. This form should not be filled out by the student. Most of our busses are full there for Bus Passes should only be requested in the case of a true emergency. "Staying with a Friend" is not an emergency. Thank you for understanding!

It is preferred to have a request AT LEAST 1 day prior to needing a pass.

FOR THOSE WHO NEED A PASS ON THE SAME DAY:

BUS PASSES WILL NOT BE GIVEN TO STUDENTS IF REQUEST IS SUBMITTED AFTER LUNCH. CALLS AFTER THIS TIME WILL ALSO BE DENIED. Thank you for your understanding!
