National Implementing Partner Application
Executive Summary
Faithful Families Thriving Communities (Faithful Families) welcomes applications from organizations with the interest and capacity to deliver Faithful Families programming in their community. By becoming a National Implementing Partner, you will be recognized as part of Faithful Families' national network of researchers, Extension programs, public health professionals, faith leaders, and lay members supporting communities of faith around health.

Please note that this not an opportunity for funding, but an opportunity for partnership as detailed below.

Our Mission & Vision
Faithful Families' mission is to inspire faith communities to be health champions for individuals, families, and local communities.

Faithful Families believes that communities of faith are important health advocates, particularly in communities affected by health disparities or lacking access to crucial resources. That’s why the program hopes to empower faith communities as they become change-makers in helping their communities thrive – physically, mentally, and spiritually.

Our Approach
Faithful Families recognizes that faith-based organizations offer particular resources that can greatly support their community’s long-term health, particularly in communities that experience the greatest health disparities. The program connects a community’s faith or religious tradition with messages about long-term health at multiple levels of the socioecological model – individual, interpersonal, organizational, and community. Faithful Families trains and brings together facilitators (nutrition and health educators) and Lay Leaders (non-clergy members) to co-deliver the program in faith settings. The program is open to all faith and religious traditions.

Over the course of the program, Faithful Families engages each faith community in:
1) Direct peer education
2) Policy, systems, and environmental (PSE) supports
3) Community engagement

Partnership Opportunity
Our National Implementing Partners model leverages the strengths of each Implementing Partner's particular strengths and assets to support faith communities in their community. Faithful Families provides Implementing Partners with training, curricula and instructional materials, evaluation tools, ongoing technical assistance, and national leadership support while Implementing Partners provide the hands-on engagement, local resources, and relationship building opportunities to initiate and sustain health initiatives in their respective communities.

Faithful Families commits to providing the following resources and ongoing technical assistance to each Implementing Partner:

1) Program materials: Faithful Families provides up-to-date curricula for partners to purchase as needed. Curricula can be purchased with an online order form. 2) In-person training: Faithful Families offers an in-person training for all Implementing Partners, which is currently held in Raleigh, North Carolina. This training prepares each partner to implement and evaluate their Faithful Families programming, and provides strategies for train-the-trainer programs in the partner's home state. It also provides a networking opportunity for Implementing Partners to share challenges and best practices. 3) Technical assistance: Faithful Families provides ongoing assistance through quarterly partner calls, newsletters, and webinars with expert panelists. Staff is also available for questions by e-mail and phone.

4) Program portal: Beginning in 2019, Implementing Partners will have access to Faithful Families' data collection, tracking, and reporting database. The portal gives each partner the ability to manage and track their Faithful Families programming.

National Implementing Partners commit to the following:

1) Attend in-person training: Prior to program implementation, Implementing Partners should attend the in-person Faithful Families National Training.

2) Program coordination: Implementing Partners should designate one person who is the primary representative for program coordination and who serves as the primary contact with Faithful Families staff. This person will be the main administrator for Faithful Families programming and will communicate with Faithful Families staff.

3) Ordering program materials: Implementing Partners will order materials from an online ordering form. Invoices will be provided per request.

4) Local implementation costs: In addition from the costs of the Faithful Families curricula materials and in-person training, Implementing Partners incur local costs to provide Faithful Families programming. Local costs include: staff time for implementation (9+ weeks of class time per faith community program, and longer term support for PSE changes and community engagement); training health educators and Lay Leaders to facilitator program; food costs of approximately $100 for recipe taste-tests for 9+ class sessions; cooking equipment for use during classes; transportation for staff; and any incentives, funding, or resources that the Implementing Partners chooses to provide.

5) Submit yearly program evaluation: Each Implementing Partner should submit a yearly program evaluation to Faithful Families. This helps Faithful Families to build national evidence and provides another opportunity for partners to provide feedback for program improvement.
