K-12 Enrollment Registration Process
WELCOME TO THE AGUSTINIAN COMMUNITY!

The enrollment procedures in schools combine all activities involved in the entire enrollment process following guidelines issued by several authorities. Due to current health situation, this might happen both online and offline, particularly in private schools, but the overall objective is to help our learners be admitted and enrolled in schools.

Applicant is advised to pay the Php1,500.00 Down Payment at the denominated payment channels (e.g. SM Bills Payment and Banks - BDO, BPI, Metrobank, Union Bank, and Robinsons Bank ).

Please prepare the scanned copies and Email to "otr-enrolrequirements@usa.edu.ph" the following:

• Payment Transaction Receipt
• 1x1 ID Picture (w/ White Background)
• Report Card
• Birth Certificate (PSA)
• Good Moral Character (GMC Certificate)

Do you have all the documents to accompany your application in your computer or mobile device? Make sure that your documents are clearly named.

Parents/Students who came to the University during the Pre-enrollment is NO LONGER REQUIRED TO FILL OUT THIS FORM. A student from other schools wishing to transfer must enroll onsite during the scheduled date.
