Louisville Street Faire Vendor Application 2019
The Street Faire will be held on the Friday evenings of June 14, 21, 28, July 12, 19, 26, Aug 2 and 9. The hours of operation will be 5:00–9:30 PM (take-down happens after 9:30 PM). New this year, the band will play until 9:30 pm where as in year prior they have stopped at 9:00 pm. The Street Faire will consist of produce, crafts, artists, bands, children’s activities, food, and a beer and wine garden that includes margaritas! On average 5,000 people visit the Street Faire every Friday night.

IMPORTANT: Only online applications will be accepted in 2019. Payments may be made via check (mail to: Louisville DBA
P.O. Box 311 Louisville, CO 80027) or credit card (3% processing charge). Once your application has been received you will receive an invoice and contractual documents, these will be provided to to you via DocuSign and Square. Please note that your application is considered incomplete until these are complete.

For ACCEPTED vendors selling items at the faire you must file a 2018 Special Events Sales Tax License Application with the City of Louisville. DO NOT send this license to the DBA, we are unable to file this license for you. The form can be found here: https://goo.gl/ckvM19.

Electricity for the Street Faire is available but EXTREMELY LIMITED and based on location. You will have to be approved and an additional fee is required. We will notify you whether your request for electricity is approved at time of acceptance. *
Insurance: Each vendor is required to provide the DBA with a certificate of liability insurance naming the DBA as an additional insured. Please indicate whether you will provide your own or plan to purchase from the DBA. *
