Healthy Communities in PA
Thank you for taking the time to fill out this short survey on you, your organization and your experiences with healthy communities. This survey will help the Healthy Communities in PA task force of the American Planning Association, Pennsylvania Chapter (APA PA) to better provide and share tools across the state that are useful and meet your needs. Healthy Communities work to link the fields of health and planning to ensure that those designing the built environment consistently consider health outcomes and those seeking to improve public health consider the physical environment. The survey should take less than 10 minutes, depending on the level of detail in your response.

Please share this survey with other partners who may be interested in providing information or participating in the Healthy Communities in PA task force. You can find information about the task force and resources on our website at https://planningpa.org/advocacy/healthy-communities-in-pa/.
