This form serves as the student registration for the Summer Academic/Enrichment Program.
Direction to parents/guardians:
Fill out the form ONLY if you want your child to attend the summer program.  Attendance is required for the full five weeks.  Deadline to complete this application is May 15, 2022.  A waitlist will be available after May 15 pending open slots. Completion of this enrollment form by the parent is committing to the whole program, understanding that daily attendance will allow their child to receive full benefits, and if there are excessive absences, that their slot may be taken by another.  If you have any questions regarding student enrollment, please call the school office.
