This survey is intended to provide an avenue for community input on the development of a harassment policy and code of conduct for The Sunstone Foundation. All answers will be kept confidential unless you volunteer any identifying information. In the event that you supply identifying information, employees of Sunstone (including the Board of Directors) will not have access to any survey answers.

All questions are optional. Your participation in this survey will not affect your current or future relationship with The Sunstone Foundation.
