🚫 The child has a contagious illness;
🚫 The child is unwilling to participate (please respect the child’s wishes);
🚫 The child requires one-on-one supervision or dedicated caregiving.
2. If the child has special needs or non-contagious conditions that requires daily medical care, please inform us proactively, such as:

3. Registration will only be processed if all required information is fully completed.
Confirmation of acceptance will be provided upon receiving the official notification.
4. The programme is free of charge (donations to support the centre are welcome).
5. Registration deadline: 31/5/2026 (or upon full capacity)
6. Dietary Requirements: Lunch and tea breaks will be provided (Vegetarian). Please indicate any specific food allergies in the section below.
7. Photography/Media Consent: Photographs and videos may be taken during the event for archival and publicity purposes (e.g., social media, newsletters). By registering, you grant us permission to use these materials
8. Emergency Contact: Please ensure the contact number provided is reachable between 9:00am and 5:00pm on the day of the event.
9. Attendance Commitment & Cancellation: As seats are limited and the program is free of charge, we kindly seek your cooperation to inform us at least 7 days in advance if your child is unable to attend, so we may offer the slot to those on the waiting list.
10. Please note that our facilitators are not able to provide 1 to 1 or the same level of support as educational assistants or behaviour therapists.
11. In accordance with the Personal Data Protection Act 2012 (PDPA), Dharma Drum Mountain Singapore is required to inform you of the following regarding the collection of your personal data and obtain your consent:
a) Purpose of Collection: To facilitate community care and outreach, your personal data (such as your name and contact details) will be used within the Dharma Drum Mountain system to provide ongoing care, support, and services through appropriate channels.
b) Your Rights: You may request to access, correct, or delete your personal data, or request that we cease the use of your information at any time.
c) Voluntary Disclosure: You have the choice to provide your personal data. However, please note that if the information provided is incomplete, we may be unable to provide you with our full range of services.

10. 温馨提醒：本项目的协调员主要负责整体活动协调，恕不提供一对一的专属陪护。协调员的角色不同于教育助理或行为治疗师，请您知悉。
11. 依据「个人资料保护法2012」规定，法鼓山新加坡取得您的个人资料，需告知下列事项，并取得您的同意：
