Registration and Costume Fee: By typing your first and last name below, you understand and agree to the following:

MIDC will automatically charge the card on file for the registration fee and extra costumes. 

Costumes will not include all tights and shoes needed for performing! 

**Hip hop dancers MUST bring their own ALL BLACK shoes.**

All costumes will be rentals and will be returned to MIDC after the performance.

Registration fee: $78 - this will cover one costume, rehearsals, videography and photography. 

Costume rental fee: $41 for each additional dance your dancer is in. 

Dancers may purchase tights and shoes through the MIDC store.

Winter Benefit – Participation Agreement

Non-Refundable Fees
Participation fees are non-refundable. No exceptions.

Payment Authorization
The participation fee will be charged to the card on file in Studio Pro on September 26, 2025.

Communication
I am responsible for checking my email for all performance communications.

Enrollment Requirement
I must be registered for classes from October 2025 through December 2025 to participate.

Attendance
If my dancer misses more than three (3) classes, they may not be able to participate in the show.

Multiple Shows
The Winter Benefit includes multiple shows; my dancer will participate in at least one show.

Age-Group Scheduling
Each age group has its own show. Siblings in different age groups will be in different shows.

Tickets
I will need to purchase tickets for each show I attend.

Mandatory Dates
If my dancer misses any mandatory dates, they may not participate in the show.

Mandatory Dress Rehearsal
There is a one-time mandatory dress rehearsal on Saturday, December 6, 2025, in the afternoon. Exact call times will be provided; attendance is required.

Closed Rehearsals
All rehearsals are closed to parents/guardians.
