This form is specifically for students in Grades 7 - 12 who are not attending Summer School but have chosen to take home their Chromebook over the summer.

This form should be used if you need help with your Chromebook, OneLogin, Google, PowerSchool or your Username & Password.

If you are going to summer school and need help with your Chromebook, please let your School know of the issues and someone will go to the school to repair your Chromebook. You do not need to fill out this form.

If you need help with REGISTRATION, TRANSFERS, ADDRESS or EMERGENCY CONTACT Changes, please contact your SCHOOL SECRETARY.
