College Counseling Family Response Form
The goal of the College Counseling Office is to support each individual student to the best of our abilities. For us to do so, we seek your input through this form, which is meant to help us understand your thoughts regarding your student's journey through NMH. We will use this information to guide our conversations with your student throughout the second semester of their junior year and through their senior year.
