OHS GENERAL VOLUNTEER APPLICATION
PLEASE NOTE: Our service area is Fremont, Newark or Union City, so volunteers must BE ABLE TO TRAVEL to these cities for volunteer shifts. Please allow 2-3 weeks for a response regarding your application.
Volunteers must be 18 years old for the Wildlife Rehabilitation Center (WRC).

IF YOU ARE UNDER 18, STOP AND PLEASE READ BELOW:
  • Students under age 18 years may be eligible to participate in youth activities and events with parental supervision as determined by each program manager.
  • ALL YOUTH (STUDENTS UNDER 18) MUST SUBMIT THE  Youth Opportunities form FIRST. 
  • GO TO THE YOUTH VOLUNTEERING WEBPAGEAFTER THE YOUTH FORM IS COMPLETE, IF THE STUDENT IS REQUESTING A PROJECT FOR COMMUNITY SERVICE HOURS, THE PARENT OR GUARDIAN MUST THEN SUBMIT THIS OHS GENERAL VOLUNTEER APPLICATION AND NAME THE STUDENT. THANK YOU

Please choose the ONE preferred PROGRAM in which you or your child would like to volunteer. Please note training will be required to volunteer for the Wildlife Rehabilitation Center, Pet Therapy, or Kitten Fostering as indicated on our Youth Volunteers page.  *

I am completing this application for my child who is under 18 years of age and whose name is entered on the line above. By completing and submitting this agreement I understand I am providing permission for my child to participate in OHS activities and that my answers I submit indicate I am legally and financially responsible for my child's volunteer services with OHS.

OHS GENERAL VOLUNTEER AGREEMENT & RELEASE
I am over 18 years old and have voluntarily agreed to serve as a volunteer for OHS, a nonprofit corporation organized under the laws of the State of California.
OHS has permission to use photographs of me (and my child named above) in materials that promote OHS or to publicize any event, without compensation or notification.
I agree to complete all required orientation, training, and documentation relating to my volunteer position(s).
I agree to disclose any physical or psychological limitations to the supervising OHS volunteer staff before participating in any activity. 
I understand I may be interacting with healthy and unhealthy animals, and may be lifting, carrying, moving, or transporting animals if assigned. I will inform the volunteer coordinator immediately of any physical limitations I may have prior to engaging in physical tasks.
I agree In the interest of the safety of the animals, staff, and volunteers that OHS has the right to revoke my volunteer privileges if these rules and regulations are not followed and that either I or OHS can terminate this relationship at will.
I agree to provide my services without compensation and understand that OHS volunteers are not classified as employees and are ineligible for Workers’ Compensation.
I agree to be legally and financially responsible and will indemnify and hold OHS harmless for my own acts and omissions relating to the service(s) I am voluntarily providing to OHS. I am responsible for providing my own medical, liability, and auto insurance while volunteering for OHS.
I understand I am voluntarily participating in the activities of OHS with full knowledge of the risks involved and agree to accept any and all risks including but not limited to that of injury, death, or damage to myself and/or my personal property as a result of my volunteer duties, jobs, assignments, and other related tasks.
In consideration for being permitted by OHS to participate in activities and provide Services, I hereby agree that I, and/or my legal representatives will not make a claim against, sue, or attach the property of OHS for injury or damage resulting from any act, omission, negligence or other acts, caused, by any employee, agent, contractor, or representative of OHS as a result of my participation in activities and performance of the services. I release OHS from all actions, claims, or demands that I, and/or my legal representatives now have or may hereafter have for injury or damage resulting from my participation in activities and performance of the services.
​I acknowledge that this agreement will apply to the entire term of my volunteer relationship and continuing as long as I continue to be a volunteer and thereafter as is necessary to protect the interests, confidentiality, and rights of OHS with respect to use of my work products without compensation.
By submitting this agreement, I admit that I have read, understood and agreed to the terms stated herein. I fully understand that I will relinquish all claims or actions known now or in the future against OHS and that I am of legal age and legally competent to sign this agreement. I am signing this agreement of my own free will without the influence of an OHS staff member.

VOLUNTEER HANDBOOK: OHS adheres to several policies to create an enjoyable and informed volunteer organization. Please read the several policies in the Volunteer Handbook linked here, then acknowledge that you have read, understand, and agree with the information in the OHS VOLUNTEER HANDBOOK  *
OHS DRIVING REQUIREMENTS: TNR, PET THERAPY, and FOSTER VOLUNTEERS ARE REQUIRED TO DRIVE. FOR ALL OHS DRIVERS,  even if you intend to drive your personal vehicle only ONE TIME to perform assigned duties for OHS, please submit the required documents to volunteering@ohlonehumanesociety.org.  Thank you.
If an employee or volunteer uses their personal vehicle for OHS business and is involved in an accident, OHS is  exposed to a lawsuit. This coverage is intended to protect the organization only, not the employee or  volunteer operating the vehicle. It also does not pay for any damages to the vehicle being driven by the  employee or volunteer. OHS will determine the limit and control the use of employee or volunteer personal  vehicles being used for business purposes

All employees or volunteers using their personal vehicle must meet the following qualifications:
❖ Must have a valid California Driver’s License
❖ May not have any major moving vehicle violations or accidents (including, but not limited to DUI, Vehicular Homicide, Suspension or revocation of license) in the past 3years.
❖ Must have a current personal auto insurance policy with a minimum of $300,000 liability or $100,000/$300,000 split
❖ Assure that the vehicle is maintained in safe working order.
❖ Driver must always wear their seat belt and any passengers must use seat belts.
❖ Driver must follow all speed limits and other driving laws(any violation of this may result in elimination of the driver’s  permission to drive their vehicle for the business of the organization and also holds the driver responsible for any  tickets, violations and/or damage).

In addition to the above, drivers who transport animals must adhere to these guidelines
❖ Animals will be kept no closer than the rear seat of the vehicle.
❖ No animal is to be transported in the open bed of any trucks.
❖ Proper temperature and ventilation will be maintained in the vehicle at all times that the animal is in the vehicle  (including when the vehicle is not being operated)

Upon hire or start of volunteer service and annually, please submit the following by email to volunteering@ohlonehumanesociety.org
❖ Copy of your current drivers’ license  
❖ Copy of your personal auto insurance policy  
❖ Order and provide a copy of your DMV driver's record report: https://www.dmv.ca.gov/portal/customer-service/request-vehicle-or-driver-records/online-driver-record-request/ 
❖ This form upon hire and annually by employee or volunteer using personal auto for OHS business.
