Intake Questionnaire prior to a Life-Coaching or EFT/Tapping Series with Christina Neyhart

In preparation for our introductory call together to see if we're a good fit for each other, please complete this questionnaire. This will help me to prepare for our call together and allow us to make the most of our time! Please ensure it's complete at least 36 hours prior to our call together, wherever possible, to give me time to review in advance.

As you sit down to complete this questionnaire, first take a minute, close your eyes, take a deep breath, and feel into your heart’s desires. This will help you to answer from your heart, which will help me feel the connection to your answers, and help us both in our call together! I’m looking forward to meeting with you!
