Parent/Student Home Textbook Request Form
Textbooks are an instructional tool and not the primary resource for classroom instruction. All textbook requests must be made through a campus administrator. If you have multiple students you are requesting books for, a new form must be completed for each student. Be certain to submit your form once you have completed the questions. Parents are encouraged to contact teachers if they have questions about whether a book for home is recommended.

It is high recommended that students use the online textbooks. Teachers can provide textbook codes to the students.

**Math and Science textbooks will be distributed through the teachers, with the exception of Algebra. If you would like a math or science book for home, please communicate with your child's teacher for that subject.
