Your Name *
Your Email *
Your Institution *
Your role at your institution: *
Are you a member of ACMS? Please note that being a member is not a requirement of hosting the Brabenec Lecturer.  *
Are any other employees of your institution members of ACMS? *
Have any employees of your institution attended an ACMS conference before? *

The Brabenec Lecturer will give one talk in the fall term and one talk in the winter/spring term with a focus on the integration of faith in the mathematical sciences.

One of the talks will be in person, but one may be virtual depending on costs and timing. What type of presentation are you willing to host?

*
The ACMS will prioritize institutions who lack the institutional funds to host a speaker on the topic of faith integration in the mathematical sciences. Does this describe your institution? *

Please indicate the dates, or date ranges, in which you plan to host the lecture if known. Dates will ultimately be subject to the lecturer's availability.

*

Please describe the nature of the event you plan to have the lecturer speak at. Please include the (approximate) number of attendees, type of attendees (faculty, students, public, etc.), location (auditorium, classroom, etc.), and modes of advertising.

Note: the ACMS will prioritize talks directed at students.

*

The ACMS has set aside $1800 to cover the travel expenses of the lecturer to two institutions during the course of the academic year. Please describe the estimated cost of having the lecturer visit your institution and which expenses you are asking ACMS to cover.

For instance, are you asking ACMS to cover flight only or flight plus lodging? Is it lodging for one night or two? What about meals? etc.

*

The lecturer may be open to the option of providing a supplementary technical talk or a faculty workshop. Are you interested in hosting a second presentation of some sort? If yes, please describe.

Please include any additional information you would like the selection committee to consider when processing your application.
