Before completing this form, please know that the school highly recommends that complaints be discussed directly with the person with which you are having conflict. We recommend that you set up a meeting with the person to discuss rather than completing this form.

After completing this form, the following steps will be taken:
1) The offending person will be notified per union contracts.
2) You will be contacted to be notified of corrective changes or to set-up a meeting for conflict resolution.

If you would like to remain anonymous, please leave the contact information section blank. However, if you do, please understand that there will be no follow-up since we will be unaware of where to direct the information.
