Bloomington Public Schools plans to shift secondary students to the hybrid learning model. Students will attend school in-person on assigned days and learn remotely on other days, with an option for families who wish to keep their students in distance learning full-time.

To finalize our plans for middle and high schools, we need every family to fill out this short survey to inform us of your plans.

IMPORTANT INSTRUCTIONS - PLEASE READ CAREFULLY:

1. Complete this form ONLY for your children who attend BLOOMINGTON TRANSITION CENTER.

2. To complete a form for children who attend another BPS middle or high school, visit https://bit.ly/secondary-hybrid.

3. PLEASE FILL OUT ONE FORM PER CHILD. There will be a link after you have completed the survey to submit another response if more than one child attends BLOOMINGTON TRANSITION CENTER.

4. Please complete ONLY ONE form per student. Make your decision BEFORE completing this form.
