I understand that St. George's Scholar Institute is providing a community service to support student(s) during the COVID-19 pandemic. I understand that St. George’s is committed to the safety of all the students but cannot guarantee other visitors or students have not been exposed to COVID-19. I understand it is my responsibility to know and understand my child(ren)’s health conditions and risk factors and take appropriate measures to protect my child(ren) from exposure to COVID-19. By signing this form, I hereby declare that I have made the decision to allow my child(ren) to attend the St. George’s Scholar Institute. I further declare and agree to waive and release St. George’s Scholar Institute from any and all liability for any injuries or damage incurred by my child(ren) that might occur while my child(ren) participates in the, unless the injury or damage was caused by willful or wanton misconduct by one of the program providers.

• I understand if my child is sick or has symptoms they cannot come onsite participate in any in-person programming until symptoms have been cleared.

• I understand if my child has been around anyone whose tested positive or has been exposed to COVID-19 they cannot come onsite or participate in any in-person programming until proper measures have been taken and cleared.

• I understand if my child has a temperature of 100 or above they cannot come onsite or participate in any in-person programming until proper measures have been taken or cleared.

• I understand that my child must wear mask while attending programming onsite unless otherwise notified by staff.
