This form is for local organisations to refer someone in the Hinckley and Bosworth area for a care package.

Packages are for individuals who are struggling, feeling isolated, or going through a difficult time.

Once a referral is received, we will confirm availability and arrange the package with the referring organisation.  

Referrals are subject to availability.
