Dear Parent,

Your child has a unique opportunity to work with a volunteer mentor from the community. These
volunteers agree to spend time each week with your child to assist them in maintaining their job or school
placement and remaining crime-free. Your support, encouragement and involvement are the key to the
mentor's success with your child. We encourage you to meet this volunteer and talk about your child's
progress.

I authorize R.A.P. Mentor Program to obtain any needed information regarding
my child from his school’s official, including academic and behavioral records and conversations with
teachers, counselors, coaches and other administrative representatives.

I agree to allow R.A.P. Mentor Program to use photographic images of my child taken while
participating in the mentoring program. These images may be used in promotions and other related
marketing materials.

By printing your name below, you agree to the above, and give permission for your child to participate. Additionally, you release the institute from any liability that may arise from your child's participation in the mentor program and all activities associated with the mentor program.
