This form is a waiver of claims/release of any legal liability on the part of Dark Ascension LARP for any injury or damages that arise from the signees participation in any Dark Ascension Event.

I hereby sign that I have agreed to take part in events run by Dark Ascension LARP, knowing fully the physical and environmental risks associated with my partaking. I am aware of the safety and consent rules, and do hereby knowingly release Dark Ascension LARP and its ownership team, as well as any sites it rents from and their ownership teams, from any liability regarding physical or mental injury or illness, damage to property, or any other legal responsibility for myself and my own personal wellbeing, as well as the wellbeing and maintenance of any goods or property I bring to Dark Ascension LARP. *

I henceforth agree that I will bear sole legal and financial responsibility for any damage to sites, goods, materials, or property, belonging to myself, Dark Ascension LARP, any site that it rents from, or any other player/event attendee that results from action I take at Dark Ascension LARP, and will not hold Dark Ascension LARP or its ownership team in any way liable for damages caused through my actions, intentional or unintentional. *

I henceforth agree that I will not bring to event or imbibe any illegal or illicit substances during my time at Dark Ascension LARP. The consumption of legal substances, such as alcohol and cannabis are permitted onsite, however, Dark Ascension LARP has a strict sobriety policy for anyone participating in combat or action heavy exploration or roleplay during events. Upon consumption of any sobriety altering substance that is not otherwise medically required, a participant must remove themselves from combat and action heavy exploration or roleplay for the duration of the game, but will still be allowed to participate in spectating and socialization. However, Dark Ascension LARP holds the right to refuse service to those who cause any problem to site or other attendees, especially regarding the use of substances at event. Be respectful to the site, the other participants, the ownership team, and the environment. *

Dark Ascension LARP and its ownership team reserve the right to refuse service to anyone at any time for any reason. I have read the above rules and hereby agree to adhere to and respect the rules of Dark Ascension LARP during all events I attend, and understand that failing to do so could result in my temporary or permanent expulsion from that event or any future events, and that I am solely liable and responsible for the outcomes of my actions. *
