1. I hereby declare that my health condition allows me to do sports in a community, I do not have any infectious diseases.

2. I solemnly declare, that:
I have not experienced symptoms similar to the flu or symptoms officially attributed to the coronavirus (i.e.: fever, fatigue, cough, muscular pain, blocked nose, runny nose, sore throat, shortness of breath, diarrhoea) on myself and there are no other reasons for a risk of infection.
- I have not visited countries or cites classified as outbreak sites,
- I do not live or work in a property locked for epidemiological quarantine,
- I have not got into contact with people involved in the above,
- I have not met anyone who is tested to be or possibly infected with coronavirus.

3. I will adhere to the rules and regulations of the sport complex with special attention to provisions on hygiene which are in accordance with the government’s rulings. I have read the figure below and I am informed about its content. I acknowledge that those who refuse to make this statement can be banned from using the sport infrastructure and participating at the trainings provided by BEAC and can be banned from entering the premises.

4. I participate at the trainings on my own responsibility. I declare that - with regard to the rules accepted in Section 3 and the possible consequences of the epidemic - the operator of the sport complex and the organiser association BEAC or its employees are not liable for the risk of infection during the use of their sport infrastructure.
