As Master and Doctoral Faculty Consultation and PhD Continuing Registration are suppressed from the Class Schedule, you will need a Class Number in order to register. Submitting this form lets your advisor know that you intend to enroll in Faculty Consultation or PhD Continuing Registration in Fall 2023 and that you are requesting a Class Number. Once you have received the class number you are responsible for enrolling by the end of the first week of classes through CiS.

Following approval by the Area Head, you will receive your Class Number via your UMail Account.

This Class Number is the property of the University of Utah and may not be shared with other students. Please register by the end of the first week of classes.
