Thank you for contributing to the body of knowledge about what the most recent food service ware practices (Pre-COVID 19) had been within K-12 schools around the country. Data collected from this survey will help CEH assist schools (including yours!) to transition to more sustainable foodware options. The results will be aggregated - your individual and organization's information will not be shared or published.

This Sustainable Foodware Sweepstakes (“Sweepstakes”) is open to all natural persons who are residents of the 50 United States and District of Columbia, who are at least 18 years old at time of entry. Void outside the USA, Puerto Rico, and other US territories. The Sweepstakes is subject to all applicable federal, state, and local laws and regulations and is void where prohibited by law.
The Sweepstakes sponsor is Center for Environmental Health, located at 2201 Broadway, Suite 508, Oakland, CA 94612. Employees of Center for Environmental Health and their family members are not eligible to win.
The Sweepstakes begins at 12:01AM on March 2, 2020 and closes at 11:59PM on May 31, 2020.

How to Enter

Limitations on Entries
LIMIT ONE ENTRY PER PERSON. No third party entry is accepted.
A total of 7 Prizes will be awarded. The prizes are as follows:
- One To-Go Ware 3-Tiered Stainless Steel Tiffin. (
- Three 27 oz Kleen Kanteen Bottles (1 bottle will be awarded to three different sweepstakes winners):
- Three Reusable Bambu Travel Utensil Sets with an Eco-Conscious Pouch (1 reusable utensil set will be awarded to three different sweepstakes winners)
Winners will be selected in a random fashion from all eligible entries collected during the Entry Period. The drawing will be conducted on or about June 1, 2020. Winner will be notified by email. Inability of Sponsor to contact winner by email within 7 days after selection will result in disqualification and another winner will be randomly drawn. Prize will be sent to the winners by U.S. mail within 14 days after winner has been notified and accepted the prize. If prize is undeliverable, the prize will be forfeited and another winner will be randomly drawn.
The odds of winning will depend on the number of eligible entries received during the entry period.
By entering this Sweepstakes, you promise to abide by the Official Rules and decisions of the Sponsor, which will be final and binding in all respects. You agree to discharge, hold harmless, and release sponsor, its affiliates, employees, officers, directors, agents, and assigns from and against any or all claims, losses, injuries, or damages resulting in participation in the sweepstakes.
