Facilities Usage Request Form
All activities and/or events must be scheduled through the church office or online at least 21 days in advance to allow for necessary staff planning and logistics around the event. For activities with 50-100 participants, the event must be scheduled at least four (4) weeks in advance. For more than 100 participants, the church requires six (6) weeks advance notice. ALL FIELDS ARE REQUIRED.
If this Request Form is for an event outside of the regularly scheduled official church activities,
a response will be provided within three business days of receipt of this form.
Fees are for events outside of regularly scheduled church business.
Facility Usage Rates: 0-3 hours $100 member/ $200 Non-member. 4-6 hours $200/$400, 7-9 hours $300/$600
