PHOTO CONSENT FORM 

I, ______________________(the “Releasor”) grant permission and give my consent to Friend of Del Mar Mesa and Del Mar Mesa Garden Club (the “Releasee”) for the use of the following photograph(s) or electronic media images that are taken during my participation in their activities for any legal use.  I understand that with my authorization below the photograph(s) may never be revoked.  I understand that I may revoke this authorization at any time by notifying the Del Mar Mesa Club Chair in writing. The revocation will not affect any actions taken before the receipt and acknowledgment of this written notification.
