Fall Designs

Please read each question and fill out your information accordingly. By filling out this form, you'll be providing the necessary information to our staff in order for them to send you the community service letter.

Receive two hours of community service per envelope completed. Limit one of each envelope per month.

Remember to return the ENVELOPE with your completed work to the Teen Center once you have finished this form!

Once BOTH the ONLINE FORM and ENVELOPE is returned and approved, a community service letter in PDF form will be delivered via email, based on the email provided in this form.

Please note that all submissions will receive a response within a timeframe of 1 to 7 days. The submission date of your community service form will be counted for the corresponding month. You must have a valid Port Jefferson Free Library card in order to receive community service credit with us.
