I request business advising services from the Northwest Innovation Hub and its affiliated programs. I agree to cooperate should I be selected to participate in surveys designed to evaluate the effectiveness of these services.

I permit Northwest Innovation Hub or its agents the use of my name and address solely for the purpose of surveys or informational mailings regarding OEN/NW Inno Hub programs and services.

I understand that all information shared will be held in strict confidence. (OEN will not provide your personal information to commercial entities.) I authorize NW Inno Hub to share relevant information with assigned coaches and Entrepreneurs in Residence(EIR) for the purpose of assisting my business. I further understand that the EIR(s) agree not to:

recommend goods or services from sources in which they have a financial interest, and
accept fees or commissions resulting from this advising relationship.

In consideration of receiving management or technical assistance, I waive all claims against OEN/NW Inno Hub personnel, and those of its partner organizations and advisors, arising from this assistance.

I self-certify that neither I nor my business have been suspended or debarred by any Federal agency. I further certify that my business does not engage in the manufacture, sale, or possession of marijuana or psilocybin. I understand that if my business does engage in these activities, I am not eligible to receive assistance through the NW Innovation Hub.

I acknowledge that I must be 18 years of age or older to sign this document.

I also understand that OEN/NW Inno Hub may leverage technology that includes Artificial Intelligence (AI) tools to assist in business advising. These tools claim to be closed-loop systems that do not use personalized information to train AI models. I recognize that I am responsible for understanding the risks and benefits of using such tools as part of the advising process.
