Family Pre-Admission Form for 2019-20 Lottery
This form must be completed for a child to be included in the lottery for the 2019-2020 school year. ONLY ONE FORM RESPONSE should be completed for each FAMILY. The form will allow you to submit information for up to 7 children. If your family has more than 7 children applying, please contact the campus and they will provide instructions on how to enter the additional children.

You must complete the entire form and press the submit button in order for your application to be complete. You will receive an auto-response that your form has been submitted. If you do not receive the auto-response, we recommend you contact the campus. It is the responsibility of the parent to contact the campus and verify receipt of the application.

Your child must be 5 years of age ON or before September 1, 2019, to enroll in Kindergarten.
