CARE Network School Team Application
The Center for Research on Equity and Innovation at the High Tech High Graduate School of Education and partners from CREATE at the University of California, San Diego and the California Math Project, are seeking middle schools to join the CARE Network. Our aim is to increase the number of students who are Black, Latinx, Indigenous or from low-income backgrounds who have a strong academic identity and are on-track in 8th grade to graduate high school and successfully enter college and career.

We welcome applications from middle schools in the Southern California region. It is our hope that this application should take no more than 15 min to fill out.

Funding for participation is provided by the Gates Foundation, and additional funds are available for substitute costs for participating teachers. We will continue to accept applications until all 6 remaining network slots are full. We will contact schools to schedule a virtual meeting after we receive the team application (this form).

One member from the team can submit the application on behalf of the school team, however, we have found that the most successful teams are those where all team members are aware of and committed to the activities and goals of the network. For this reason we are asking teams to also fill out and upload a team participation acknowledgement letter (http://bit.ly/TeamLetter) and a letter of support from their district office (http://bit.ly/DistrictLetter), in addition to filling out this application form.

All schools participating in CARE must provide initial student level data and a district signed MOU.
