In order to facilitate the issuance of your QR Card, you may please assist us to fill in this application form, which shall be used for the verification and control.

You may note that any information provided shall only be utilised for the said purpose, and is for records purpose only.

The QR Certificate will be available digitally, from 30 August onwards, you will need to access the mobile application - HJ Hospitals Mobile Application, through which using your ID and password you can download a copy of the same.
