DISCLOSURE, AUTHORIZATION, AND CONSENT FOR SOCIAL MEDIA OR OTHER ELECTRONIC COMMUNICATION INVOLVING ST. DOMINIC SCHOOL STUDENTS

To ensure the utmost transparency and parental involvement, St. Dominic School/Archdiocese of Saint Paul and Minneapolis has created this consent form. By completing this for, parent/guardians are providing SDS staff authorization to electronically communicate with their child(ren) enrolled at SDS.

I understand that such electronic communications may be made via text, email, telephone and cell phone, social media, digital networking, and other electronic means, in accordance with the Archdiocese of Saint Paul and Minneapolis Acceptable Use Policy for Electronic Communications, which can be found here: https://safe-environment.archspm.org/200-administration/201-acceptable-use-technology *

Further, I understand and agree St. Dominic School personnel are not required to share public social media communications. I acknowledge that to review or receive public social media communications are made with my child(ren), I will need to become a fan or follower of the same social media. I understand that communications may be accessible or viewable by others who are also fans or followers of the same social media.
This Disclosure, Authorizations, and Consent form is valid for one year.
If I choose to rescind my authorization and consent, I agree that I will inform St. Dominic School/Archdiocese of Saint Paul and Minneapolis in writing and that my rescission will not take effect until it is received by St. Dominic School/Archdiocese of Saint Paul and Minneapolis. *
