Volunteer Application
Any person wishing to volunteer in a capacity with students must complete the Volunteer Clearance Process prior to being assigned to a volunteer position within a building. The information disclosed on these forms will be used only in making the initial decision of whether you are eligible to volunteer and will not be used or disseminated for any other purpose. This process includes the following documents:
  • Disclosure Statement. Form required to comply with current RCWs.
  • Immunization and Infectious Disease Record. This information is gathered for the protection of students, staff and community members in the event of an outbreak of disease. Marking Personal or Religious exemption may exclude a volunteer from work during the duration of the outbreak.
  • Washington State Patrol Request for Criminal History Information. Information on this form is used to obtain a background report pursuant to current RCWs and serves as notice to the applicant that the background report will be requested. Background reports received from WSP are maintained as confidential records in the District Office. Decisions in regard to an applicant’s status as a volunteer are made following receipt of the background report.
  • Guidelines for Volunteers and Chaperones. This signed document provides basic information on procedures for the Volunteer and Chaperone.
All of the information I have provided in this application is true, correct, and complete. As a volunteer for the Ridgefield School District, I understand that the services I am providing are voluntary in nature, and I do not expect to receive any compensation or benefits from the School District. I understand that volunteering depends on the truth and accuracy of the information provided on my application, the disclosure statement and results of the background check. I understand the Ridgefield School District has the right to determine who is accepted as a volunteer.
