Perkins Inventory Tags Order Form
DO NOT simply "order a bunch" so you know you have plenty.  This is a luxury option the CTE team is offering to entities.  Please be thoughtful in your orders, order only what you need.  Nothing more.  We would prefer to send a second batch than mail out too many initially and have them go unused.

Have you reviewed your Perkins Inventory to determine the actual number of tags needed?   *
What entity are you requesting them for? Note:  If you are in a consortium, contact your consortium director for inventory tags.   *

How many 'Leading X' inventory tags are you requesting?  For items purchased prior to FY25. *
How many 'Leading 25' inventory tags are you requesting?  For items purchased with FY25 Perkins budget funds.   *
Inventory Tags will be provided in the order received.  If for some reason we run out, more will be ordered but there may be a slight delay.  Mailed inventory tags may take a few weeks for processing.   *
A copy of your responses will be emailed to the address you provided.
