By completing this form and subsequent payment form by Go Cardless, you will be signed up to lessons with Up Beat Drum Academy so you can receive your initial lesson!

If after your initial lesson you really aren't sure it's for you, then you can withdraw from lessons without incurring any cost, you will need to email us at least 24 hours before your second lesson, lessons will cease and you will not be charged for anything.

However if you do enjoy your initial lesson, then we will set up your payments and you will continue lessons, the complete details and our full terms and conditions can be found later on in this form.
