Parent/Student Permission to Drive Separately Form
Parent/Guardian Permission for a Student to be a Passenger in a Private or
Rental Vehicle
It is occasionally necessary for a student to be transported to or from school activities in a private or rental
vehicle being driven by an authorized parent or other adult driver. The following requirements apply for
authorized use of private or rental vehicles:
1. The principal must approve any activity for students who will need to leave the school campus.
2. Each driver must have a valid Utah driver’s license in order to qualify for transporting students.
3. Drivers may not drive if they had had a conviction in the past 10 years for an alcohol/drug
related driving violation or more than two moving violations in the last 12 months.
4. Drivers must have liability insurance coverage on their vehicles as required by state law. The
district does not assume this responsibility. A current, valid insurance identification card must be
carried in the vehicle at all times.
5. Drivers must ensure that the vehicle has passed state-required safety inspections.
6. The driver and all passengers must wear seat belts.
If you consent to your student to be transported in a private or rental vehicle being driven by an
authorized parent or other adult driver who has agreed to comply with the above requirements, please
complete this form and return it to your child’s school 24 hours in advance of the event. If you do not
complete this form and submit it 24 hours in advance of the event, your student will not be allowed to
be transported in a private vehicle.
Please note that in the event of an accident or injury, the driver’s insurance will be the primary coverage
for liability. If you would like information of the person driving the private vehicle (name, contact
information, insurance information, etc.) with whom your student will be riding, please contact the
school and we will provide that information to you.
