This form is to apply for your club/society to become affiliated with UDUK

This form should only be filled out by the president/chairperson of the club/society.
Please use an email address affilaited with your university or student union where possible. (your personal .ac.uk email address is fine if you do not have an official club/society email)

By filling out and submitting this form you agree to University Darts UK(UDUK) storing your infomation and using it to communicate with you and your ratifying body.
You also agree to follow and enfoce the rules laid out by the rules and regulations documentation available on the UDUK Website.
You also understand that your student members will have to register with UDUK before they are able to compete in your teams.

What Happens Next?
You will be contacted by the committee letting you know the status of your application process.
Any news that will effect all members or applicants will usually be posted to the UDUK website.
If you do not hear anything after a while then you can contact committee@uduk.co.uk yo follow up on the process and see where it is at.
