Meal Data Reporting
This form is used to collect distribution meal data. Please note, there is NO requirement for submitting data, your participation in doing so is completely voluntary.

That said, the submission of meal data is extremely important for the City, as they use this information to assess which areas in the city receive high-turnout vs. low.

Meals Served
A few tips when completing this section:

1] For total number of meals served, simply add the number of breakfast and lunch meals together.

2] If you provide 6 meals per student (e.g. 3 breakfast, 3 lunch), the number of total meals will be divided by 6 to calculate the number of students served. This is applicable to any amount of meals served, per student.

*Provide an estimate if necessary. Numbers should be whole.
