Class of 2025: FINAL Transcript Request Form
Lake Brantley HS will send ONE (1) Official Final Transcript for each student to the College/University the student plans to attend.  

If you need a transcript for any other reason (Scholarship, Job, Military, etc.) you will need to visit Lake Brantley High School during summer hours to pick up a hard copy transcript after May 28th.

DO NOT SUBMIT A DUPLICATE REQUEST TO THE SAME SCHOOL. Please note: It can take up to 72 hours for the colleges to receive our transmissions depending on the day of the week you submit your request. The colleges often leave them unread until you call the admissions office.
**This means you will need to contact the admissions office of your school to verify receipt of your transcript.
LAKE BRANTLEY HIGH SCHOOL WILL SEND/PRINT ONLY ONE TRANSCRIPT FOR EACH GRADUATING SENIOR. Be sure you request it to be sent to the correct school.
Multiple entries will not be honored. The first entry made is where the transcript will be sent.

I understand that the SCPS district office will not finalize senior records in skyward until   
June 6, 2025.  Therefore the earliest Lake Brantley HS can begin sending Senior Official Final Transcripts will be June 9, 2025.  Lake Brantley staff will make every effort to get transcripts sent as quickly as possible starting June 9th.
