RPC Online Screening
Only one adult per family is required to complete this screening on behalf of the family.

We will use the information below only for the purposes of administering the COVID-19 screening process and complying with measures that are stipulated under the Reopening Ontario (A Flexible Response to COVID-19) Act, 2020 and occupational health and safety legislation.

Please complete this Online Screening tool BEFORE leaving for camp.  If you are bringing any guests with you who are not immediate family they must complete this screening tool independently.  Send your booking ID and cabin information to the person completing the screening.  If a family member is arriving on another day they will complete this form on their day of arrival.
 
If you or anyone in your family answers YES and has ANY symptoms that are out of the norm (meaning not due to allergies etc.) and listed below DO NOT come to camp and contact a screening location to get tested for Covid-19.

Have any of you been identified as a "close contact" of someone who currently has COVID-19 or has tested positive in the past 5 days? *
In the past 14 days have you been directed by a federal border agent to comply with federal quarantine requirements due to international travel? *
