Waiver Form: Maunalua Fishponds
Mahalo or joining us in stewarding Maunaluaʻs precious springs and fishponds: Kānewai Spring in Kuliʻouʻou and Kalauha‘iha‘i Loko Iʻa in Niu. Please fill out our volunteer waiver form below. 


A  waiver form must be filled out for each participant. A parent or legal guardian must complete a form for each minor. The information you provide will not be shared but will be compiled anonymously for internal purposes. We are grateful for every volunteer and for the tremendous support provided by this inspiring community.

I/We also understand and acknowledge that there are inherent dangers and risks involved with participation in the above workday with Maunalua Fishpond Heritage Center, which include, but are not limited to: possible exposure to heat and strong sunlight, possible exposure to cool, windy or rainy weather, walking across slippery surfaces and uneven surfaces or rough ground, possible danger of falling on rocks, possible injury from falling branches, coconuts, etc., possible encounters and bites/stings with spiders, centipedes or insects such as mosquitoes, bees, or wasps, possible injuries from contact with soil, water, plants and tools, including possible exposure to pathogens (such as leptospirosis) that are found in Hawaii's streams and soils including project work related elements.
I/We understand that I/we should be covered during the workday above by a private medical and liability policy; and I/we further understand that Maunalua Fishpond Heritage Center does not provide such insurance or otherwise indemnify individuals with respect to injuries or other liabilities arising out of participation in the above workday.
In consideration of being permitted to participate in the above workday, I/we hereby agree to assume all risks and responsibilities surrounding my/his/her/our participation. I/We have read and understand any and all written materials setting forth the requirements for participation in the above referenced activity, as well as those explained by the instructor(s), and I/we agree to strictly observe them. Further, I/we do for myself, my heirs, executors, and administrators hereby accept full responsibility for my child’s participation and agree to indemnify, release and discharge the Maunalua Fishpond Heritage Center, its officers, directors, employees, agents, volunteers and assigns from any and all claims or actions for property damage, personal injury, and/or death arising from such participation in the above workday or growing out of or caused by any acts or omissions of the above named child during their participation in above workday.
I/We hereby waive all rights or claims in regard to invasion of the right of privacy, right of publicity, and I hereby waive all rights of inspection of approval and irrevocably release Maunalua Fishpond Heritage Center and its agents from any liability arising out of or in connection with the use of my name and photographic likeness.

REMINDERS + RESOURCES
As a reminder, you will be visiting a natural resource site, so please dress accordingly. We recommend volunteers wear covered shoes, sun protection, and a hat. Long sleeve and long pants are optional.


 Please check with your group's coordinator for parking instructions after registration is complete, and provide a screenshot of this waiver if your coordinator requires it. 


The Maunalua Fishpond Heritage Center is a 501(c)(3) non-profit organization working to protect, preserve, and restore remaining Hawaiian loko i'a, springs, and cultural sites in Maunalua. The heritage center manages two sites: Kānewai Spring in Kuli'ou'ou and Kalauha'iha'i fishpond in Niu, caring for the native species, community members, and connectivity that feed and flourish in these wahi pana.
