Group Registration for Spring Youth Retreat at Montgomery 2023
This registration form is intended to be completed by a chaperone/leader for your church group. Please fill in the information as you receive it to maintain accurate attendance projections as we plan for this retreat. We want to ensure that we do not exceed capacity, and need your support for the most up to date information.

"The Trail Mix of Life"

Join us April 21-23, 2023 for Camp Montgomery's Spring Youth Retreat! 

Trail mix is a blend of sweet and salty, just like life! All 6th to 12th graders are welcome to attend Montgomery Center for a time of fellowship, camp activities, service projects, and late night campfires!

All are welcome! Please coordinate with your local church to complete registration. Adult chaperones are required for all groups. The suggested ratio for this retreat is 1:10.

Youth Participants: $125
Adult Chaperones: $75
Help offset the cost of t-shirts: $5 per participant
All registrations require a $25 non-refundable deposit to secure your reservation.

I understand that registration closes on April 7th, and I will need to turn in an updated group roster if different than the one turned in on April 3rd. After April 7th, I will need to reach out to Montgomery for any last minute registrations to check availability.  *
I understand that my group is responsible for collecting and bringing all necessary medical forms, emergency contact information, and the administering of any prescribed medication for everyone in our group. I also understand that my group is responsible for bringing first aid supplies and the administering of those supplies if needed.  *
*I release all rights to any photos or video tapes made of my child or myself for public relations or camp promotional purposes. Please indicate on the group roster that is turned into Montgomery any individuals that this statement does not apply to. *
