This form must be completed FOR EACH CHILD that rides the bus to and/or from Summit Academy North hub locations. Your Child MAY NOT ride the bus until a bus has been assigned to them, and you have received a email or letter with the information of times and location. We require 24-48 business hours to put your student on a bus.

Delivery Instructions *

Any student in sixth grade and above will automatically be released from the bus when they arrive at the bus hub. Please check the appropriate box below for your student. Any student in fifth grade or younger MUST have a designated pick-up person at the driver's bus door to release them. Summit Academy North reserves the right to refuse the release of any student to an unsafe environment upon delivery. If your fifth grade or below child is to be released to walk home, you must indicate this by checking the appropriate option below.

5th grade and below: My student may get off the bus ONLY WITH other older family members.

My student is 6th grade or above and doesn't need parental approval to exit the bus.

5th grade and below: My student May get off the bus alone at the hub.

5th grade and below: My student may NOT get off the bus alone at the hub.
