Thank you for taking the first step to enroll in a high quality early learning program / service. Follow the steps below to learn more about enrollment options and services for Preschool children.

You may complete this application online or visit the Vivian Adams Early Childhood Center and get a printed application to complete and submit in person. When you and your child visit the school for the Enrollment Screening Interview, please bring the following documents so our staff can add them to your child's Student File:

- Child's official Birth Certificate (obtained from County Court House, not the hospital)

- Child's current / up-to-date Physical and Immunization Record

- Current / up-to-date Dental Exam Records (or schedule a dental exam during your visit at the school)

- 2 Proofs of East St. Louis Residency (select 2  - such as apartment / house lease; mortgage document or property deed; voter registration card; valid driver's license; State medical card; Utility bill for gas / water / electrical service)

- Income Verification (2 current paystubs, or SSI statement, or TANF statement, or W2 / 1040 Form
