Welcome to the Tennessee AWARE School-Based Mental Health Referral Pathway! The Referral Pathway is a tool to help students and families access mental health counseling and support services in their schools and communities.
For students, parents, or school staff:
Select either the STUDENT SELF-REFERRAL FORM or the SCHOOL STAFF/PARENT REFERRAL FORM option below to request mental health counseling or student supports for any student who may be experiencing emotional or behavioral health challenges.
For Service Coordinators or School/Community Providers:
Select the SERVICE COORDINATOR/PROVIDER option below to update records in the referral pathway for any student with a previous service request.
This pathway will be monitored closely by the school Service Coordinator; however, this review may not be immediate. If you are a student in crisis (or are worried about a student in crisis) in need of immediate assistance, you can text the Crisis Text Line at: 741-741, call the Statewide Crisis Phone Line at: 855-CRISIS-1 (855-274-7471), or call the National Suicide Prevention Lifeline at: 800-273-TALK (800-273-8255).
