OHFUMC Worship Registration Form
Moving into Phase 2.1 of opening, OHFUMC may now have up to 25 people attend indoor gatherings, including worship. Because of the limited number of attendees, we are asking that you pre-register to attend worship in person. Registration will be available for four Sundays at a time.

Safety Screening and Procedures
Help us keep our congregation safe by following the safety procedures for our church and staying home when you're not well.
Have you or anyone in your household experienced any of the following symptoms in the last 14 days? If so, please do not attend any activities or events in person and wait until you have been symptom free for 14 days before signing up to attend worship in person. This question will be asked again when you check-in for worship in person.

Muscle aches or body aches
Vomiting or diarrhea
New loss of taste or smell

Required
All worship attendees will have their temperature taken prior to entering the church building. Anything above 100.4 is considered a fever, and those people will be asked to go home.

We are accepting registration requests for four weeks at a time. You may select more than one date, but depending on how many people are interested, you may not be approved for all dates. All services start at 10:00 am.
