Community Service Documentation Form
The following form is required for any hours to be documented on your transcript.  Only activities that have a valid preapproval and documentation will be tabulated.  Please remember to print before you hit submit.  Thank you for all the community service you have provided!

Provide a first and last name.  The supervisor can never be a student. A parent may not be a supervisor unless the parent is in an official role in the organization.

Provide a phone number and/or a valid email address.  If we cannot contact the supervisor, we cannot verify your hours.
Number of hours completed.  Please round to the nearest 1/2 hour. *

Did you submit a preapproval form for this activity? *
The preapproval form must have been received before the activity started (even a minute before if necessary).
