This form includes registration, payment information, and (if needed) financial aid request info for one adult attending Art Camp 2018. Please complete this form by August 15th. If you are requesting financial aid, complete this form as soon as you can to receive the aid you need.

I will be driving and I can offer rides.
I will be taking public transportation and need a ride from a station in Portland or Augusta.
If possible I'd like to get a ride from my home area.
Please note ALL dietary needs here and our cook will do their best to meet them.

I prefer to stay in a cabin with people of the same gender as me.
I would be happy to stay in a mixed-gender cabin.
I live locally and plan to sleep at home.
