Incident Reporting Form 
The purpose of this form to report any harassment or concerns of JAFAX attendees. Per JAFAX 28 Rules and Guidelines Harassment of any kind is prohibited. Harassment includes but is not limited to:

  • Intimidation, stalking, or threats
  • Discrimination based on race, ethnicity, nationality, gender identity, sexual orientation, religion, disability, or appearance.
  • Sustained disruption of talks or other events.
  • Offensive language, gestures, or imagery.
  • Photographing/recording someone against their expressed wishes.
  • Sexual harassment or unwanted attention including physical contact.
  • Belittling one’s fandom or expression thereof.
  • Disrespectful, dismissive, or offensive verbal comments related to any of the above.
