The School Health and Nutrition Unit is temporarily disabling this form for APE. As of writing, CGH has only scheduled the division office personnel for the executive check up.  Other deped personnel shall be scheduled for next year or earlier (subject to change c/o Camiguin General Hospital).  Memo to follow.
